SUBSTANCE ABUSE EVALUATION
This evaluation is designed to identify cognitive, affective, and behavioral factors that reflect a maladaptive pattern of substance use contributing to impairments in one’s ability to fulfill major role obligations, or recurrent social or legal problems. It consists of a clinical interview, review of relevant background information, and application of standardized assessment instruments. Individuals are classified according to the level of severity of their problem, and recommendations are made matching their treatment needs with appropriate interventions. All clients will be required to come in person to one of our three locations for their intake appointment.

What to Expect:
Your evaluation will last approximately 60 minutes. The clinician will call you at the appointment time that was provided at your intake appointment or at the time provided to you by the office staff. Please ensure you notify the office of any phone number changes.
