MENTAL HEALTH EVALUATION
The Mental Health Evaluation is a psycho-social assessment which consists of a clinical interview, review of relevant background information, and application of standardized assessment instruments. It is designed to determine cognitive, affective, and situational factors that may interfere with an individual’s ability to cope with or manage situations. The evaluation is completed by a licensed mental health professional, and examines the individual’s mental status, current level of emotional and social functioning, and need for treatment. Following the completion of the interview, a report is generated that summarizes the significant features of the case, and a corresponding treatment plan. All clients will be required to come in person to one of our three locations for their intake appointment.

What to Expect:
Your evaluation will last approximately 60 minutes. The clinician will call you at the appointment time that was provided at your intake appointment or at the time provided to you by the office staff. Please ensure you notify the office of any phone number changes.
